Our Mission

For decades, Elevate NWO has provided evidenced based HIV and hepatitis C testing, treatment, harm-reduction services, and comprehensive wrap-around supports that respond to the changing needs of our community, from housing to mental health and beyond. We support with compassion, dignity, and evidence-informed care

With guidance from a volunteer Board of Directors, we focus on community-driven, evidence-based responses that meet people where they are and walk alongside them toward better health and well-being.

Our Board

This dedicated group of volunteers, made up of people from our community who genuinely care. They are responsible for governance and strategic planning for the organization..

Board members include people with lived and living experience, alongside individuals working in healthcare, social services, and community support roles. While they come from different backgrounds and represent a range of priority communities, they share one common goal: a deep commitment to the well-being of Elevate NWO’s members and the community we serve.

Strategic Plan

Over the past year, staff, peers, participants, leadership, community partners, and supporters contributed their insight, honesty, and vision. Their voices have shaped a plan that reflects real needs, real challenges, and real hope for the future. 1 Respond to Gaps and Support Community Stability

Over the next five years, Elevate NWO will strengthen and expand its HIV, Hep C & Harm Reduction treatment and wrap around supports to respond to the growing needs around toxic drug supply, mental health, primary healthcare, and social isolation. This work begins with building a strong foundation.

Peer leadership is essential to Elevate NWO’s identity and impact. Over the next five years, the organization will deepen its commitment to ensuring that people with lived experience guide programs, policies, and community initiatives

Health equity requires strong partnerships, shared understanding, and the willingness to address the root causes of poor health, such as income, housing, culture, and belonging.

4 Strengthen Organizational Health and Sustainability

To remain effective, compassionate, and responsive, Elevate NWO must nurture its internal health. A strong and well supported organization is essential for delivering consistent, highly relevant care, especially in a landscape where community needs continue to grow and shift. When staff feel valued, grounded, and supported, they are better able to offer the dignity, connection, and stability our participants rely on. Strengthening our internal systems, governance, leadership, financial sustainability, and workforce wellbeing ensures that Elevate can adapt, innovate, and continue showing up as a trusted partner in community health.
