Community Notice: Medical Transportation
Medical Van:
- Must be booked 24 hours in advance.
- Can be booked Monday to Friday between the hours of 9:00am-3:00pm.
- Must be a registered member of a First Nation for the service.
- Methadone/Suboxone clients are eligible to attend Doctors appointments and urine screening/samples only.
- All appointments will be verified by Fort William First Nation prior to services being provided.
- Transportation will only be provided to the members that have the appointment, unless they are under the age of 18 years.
Non-Urgent Medical Transportation (Taxi Slips)
- Non-urgent transportation (taxi slips) are available (4:00pm – 8:00am Monday to Friday and 24 hours on weekends and Holidays).
- Taxi slips will only be issued to the Thunder Bay Regional Health Sciences Centre.
- If there us no answer to the call, please leave a message with the following information: Name, address and a phone number to return your call.
- Taxi costs will not be reimbursed.
- Anyone that receives taxi slips will be required to return a confirmation slip to the FWFN Health Centre that is signed by the doctor seen at the emergency. If confirmation slips are not returned to the office, services will be suspended.
Medical Travel out of the Thunder Bay Area:
Any registered member that requires medical travel out of the Thunder Bay Area are required to submit the following information:
- Letter from referring Doctor.
- Appointment letter from the doctor being referred to, stating the date, time, and location of the appointment. If an escort is required and the length of time that the member is expected to be gone.
- A confirmation of attendance at your appointment is also required upon return from your appointment.
This information is required to be submitted a minimum of two weeks prior to travel. Any delay in getting this information into the Health Department may delay payments to the member and escort.
