Our Collaborative Quality Improvement Plan (cQIP) drives coordinated action across our Ontario Health Team. In partnership with local providers and organizations, we are working to improve the quality of care, strengthen system integration, and achieve better outcomes for patients and caregivers across the region.

Each initiative led by our working groups contributes to advancing key cQIP indicators. Current areas of focus include:

  • Reducing emergency department visits as a first point of contact for Mental health and addictions by strengthening access to primary care and mental health supports.
  • Decreasing alternate level of care (ALC) days through improved coordination between hospital and community teams, ensuring smoother transitions and clear care pathways after discharge.
  • Improving cancer screening rates for colorectal, cervical (Pap test), and breast (mammography) cancers by identifying gaps, increasing outreach, and supporting providers to embed screening into routine care.

Through shared goals, data-informed decision-making, and community-driven priorities, our cQIP work is building a more responsive and connected healthcare system.
